Enrollment for all grade levels will be on May 18-23, 2015.
Requirements:
Form 138 (Report Card)
Prescribed haircut (for boys)
For incoming Grade 7 and transferees:
Birth Certificate (photocopy)
Certificate of Good Moral Character
Procedure:
1. Present requirements to the enrolling personnel.
2. Fill out Tentative Enrollment Form (TEF).
2. For incoming Grade 7 and Senior High School, proceed to the picture-taking area to have your picture taken for the ID. After which, proceed to the Reading Test area and take the Reading Test.
3. Present TEF and Requirements to the EMIS personnel for electronic database registration.
4. Submit TEF and all requirements to the enrolling personnel for listing and sectioning.
Regular classes start June 1, 2015.
